New Patient Registration Process
To become a registered medical cannabis patient in Hawaii, please complete the following steps:

Create your account through the Hawaii Department of Health Medical Cannabis Registry using a valid email address.

Complete the online application and upload one of the following:
Valid Driver's License
State-issued Identification Card
Valid U.S. Passport

Submit your payment directly to the State of Hawaii using:
Credit Card
Debit Card
Electronic Checking or Savings Account

After submitting your application, contact Glow Health and Wellness Center, LLC to schedule your certification appointment.
Initial patients must establish a patient-provider relationship and complete an in-person evaluation with our provider.
Please complete all Glow Health and Wellness Center intake forms before your appointment whenever possible. If forms are completed in the office, please arrive 15–20 minutes early. -
Your qualifying medical condition will be evaluated by our certifying APRN.
If approved, your certification will be submitted electronically to the Hawaii Department of Health for review.

Applications are processed by the Hawaii Department of Health in the order received.
Once approved, you may access and print your Hawaii 329 Medical Cannabis Registration Card.
Please note that the State—not Glow Health and Wellness Center, LLC—will notify you when your application has been approved.
If corrections are required, the State will return your application electronically for revision and resubmission.
Renewing Your Hawaii 329 Registration
Renewing your registration follows a process similar to your initial application.
Please contact Glow Health and Wellness Center, LLC to schedule your renewal appointment.
Patients may be eligible for a two-year registration if:
They are renewing with the same certifying physician or APRN.
Their condition is determined to be chronic.
The certifying provider believes a two-year certification is appropriate.
Please discuss your eligibility with your provider before submitting a two-year renewal application.
All certification fees are non-refundable.
Patient Responsibilities
Patients and caregivers are not authorized to possess or use medical cannabis until they receive an approved Hawaii 329 Registration Card from the Hawaii Department of Health.
Whenever you are in possession of medical cannabis, you must carry:
Your valid government-issued photo ID
Your valid Hawaii 329 Medical Cannabis Registration Card
Registration cards that are altered, falsified, modified, or used by another individual may be revoked, and access to licensed dispensaries may be denied.
Medical cannabis may impair judgment, coordination, and reaction time.
Never drive or operate machinery while under the influence of medical cannabis.
Helpful Reminders
✔ Renewal reminder emails are sent approximately 60 days before your registration expires.
✔ Renewals may be submitted up to 60 days before your expiration date.
✔ Renewing early helps prevent any interruption in your registration.
✔ Your new registration becomes active only after your current card expires.
✔ Only one active Hawaii 329 registration card is valid at any time.
